What Happened: The worker went to the facility’s control room and found the circuit breaker that was thought to power the conveyor and turned it off. However, the technician did not have an electrical panel lockout kit on hand and skipped applying a physical electrical breaker lockout device. Instead, a temporary note stuck on the panel’s cover was used.

The Problem: The worker went inside the mechanics of the conveyor for some adjustments to its drive chain. Then, the supervisor came to the panel and turned on the circuit breaker. This is because he thought the notice was outdated and saw that there was no electrical lockout switch on the breaker’s handle.

The Incident: The conveyor was activated. This caused the worker to suffer from multiple broken bones as a result of being crushed between some components. An investigation revealed that the company violated LOTO rules (OSHA 29 CFR 1910.147 and similar) regarding hazardous energy isolation.
