#1: CSB Investigation Report: Toxic Chemical Release at the DuPont La Porte Facility (Report No. 2015-01-I-TX)

On November 15, 2014, some process complications contributed to the blockage of pipelines due to the accumulation of methyl mercaptan slurry. Efforts were made by the employees to unblock the pipelines.
